To Whom It May Concern:

For the next fifteen months, our family will be in great financial distress.  During this period, I will be notably late on many payments for services and goods.  The principal reason is that my wife is emerging from an eighteen month long coma and that she is no longer able to work.  Besides both of us working, she handled the home finances.
During this period, I request that essential services and goods will be maintained, that late charges and penalties held in abeyance, and that you will make any possible downward adjustments in cost to us in the normal course of you doing business.
Essentially, she has been abandoned by both her HMO and the State of California.  I've enclosed a brief document which chronicles this abandonment.
Finally, after more than six months, she has a new primary care physician who can certify her medical condition.  If you need this, please let me know.
I pray that you will understand and assist us the best you can.

Sincerely,
